Summer 2010 Application Forms

APPLICATION forms are due back to Horizons by February 12, 2010. Application forms are to be filled out completely and signed by parent or guardian.

A STUDENT EVALUATION form must be completed for each student and completed by the student's guidance counselor/teacher and submitted by February 12, 2010.

Summer 2010 Enrollment Forms 

Once a student is admitted into the program, each student must submit the following by March 31, 2010:

• Virginia Beach ENROLLMENT FORM (Chesapeake Bay Academy)
• Norfolk ENROLLMENT FORM (Norfolk Collegiate School)
• Portsmouth ENROLLMENT FORM (Portsmouth Catholic Regional School)
• PAYMENT is due at this time
• RECORDS REQUEST FORM to be completed by the guidance counselor
