2008 Application Forms

APPLICATION forms are due back to Horizons by March 19, 2008. Application forms are to be filled out completely and signed by parent or guardian.

A STUDENT EVALUATION form must be completed for each student and completed by the student's guidance counselor/teacher and submitted by March 19, 2008.

2008 Enrollment Forms 

Once a student is admitted into the program, each student must submit the following by May 2, 2008:

• ENROLLMENT FORM
• TRANSPORTATION FORM (Norfolk Collegiate, Cape Henry Collegiate, Portsmouth Catholic Elementary)
• PERMISSION FORM
• Payment
