Application questions should be directed to your public school guidance counselor or to Tamara Turner (Horizons Program Director) at (757) 412-0249 option 3.

The Summer 2012 Application deadline has now passed.

Application forms should have been filled out completely and signed by parent or guardian.

Student evaluation forms should have been completed for each student and completed by the student's guidance counselor/teacher and submitted to Horizons.

Summer 2012 Enrollment Forms are coming soon! Once a student is admitted into the program, each student must submit the following by March 31, 2012:

PAYMENT will also be due at this time
RECORDS REQUEST FORM to be completed by the guidance counselor
